A dark lower leg would be in most cases direct evidence of chronic changes in the skin due to long standing venous insufficiency.  Chronic venous insufficiency (CVI) is usually associated with severe cases of varicose veins.  A dark lower leg could also be related to patients with history of blood clots of the deep veins.  These deep vein clots or DVT (Deep Vein Thrombosis) sometimes could remain undetected for many years or could be caused by an injury to the lower extremity. Darkening of the skin is usually accompanied by hardening of the tissues on the lower leg, chronic swelling that could progress, if left untreated to a leg sore or ulceration.
